Well-being - Pathways to Health: Penwith
West Cornwall Healthy Living Centre’s Pathways to Health & Wellbeing project aims to empower and support individuals from deprived communities and families, people with low level mental ill health and people aged over 50 in West Cornwall to overcome their barriers and assist them to progress onto the pathways of health and wellbeing via a range of wellbeing activities, including lifestyle mentoring/coaching, self-esteem, lifeskills and confidence training, counselling, volunteering, physical activities/exercises and healthy eating/cooking.
The project aims to work in partnership with GPs, health professionals and voluntary and community organisations/groups to support individuals to encourage them to make a positive change in their lifestyle. We work with GPs to develop the GP Recommendation Scheme in order for GPs and health professionals to recommend patients to relevant lifestyle/exercise/wellbeing activities organised/delivered by local VCS organisations.
We plan to install our database and website in GP surgeries to increase access to wellbeing information and activities for local people to improve their health and well-being in a more holistic and proactive way. We recruit volunteers as ‘health champions/heroes’ and ‘buddies’ to provide a network of support for individuals in order to promote positive lifestyle; and also to improve confidence and self-esteem for individuals through volunteering.
