The SWWB Programme (2013-2015) is a collaborative approach to promoting health and well-being within local communities through locally based services. The programme covers eight community based partners in the South West and three pilot diabetes interventions ‘Living well Taking Control’ in the South West with extensions in the West Midlands and North East.
An external evaluation of the programme was undertaken by the University of the West of England (UWE), Bristol and can be viewed here along with a series of Social Return on Investment reports.
