Diabetes Prevention Programme Coordinator Rate of pay: £28,000 Hours per week: 37 Location: Based in Somerset for part of the week and Dorset for part of the week with travel across both counties Westbank is delighted to have been awarded the contract to deliver Healthier You the pioneering Diabetes Prevention Programme for the NHS in Dorset and Somerset STP. The ground-breaking programme aims to reduce people’s risk of developing Type 2 diabetes by supporting people to make sustainable lifestyle changes that help them to better understand and to take control of their diet and exercise and to better manage their health. Overview of Role: As the co-ordinator you will be key to the success of this programme, managing the programme roll-out across delivery sites in Dorset and Somerset. You will manage a team which will work closely with GP Practices, other clinical stakeholders and community leaders to increase participation in the programme particularly by harder to reach groups. You will require excellent leadership and communications skills presenting to and engaging with Primary Care Teams, other health professionals and wider partners to ensure successful referral into the programme. We are looking for a confident and professional individual with experience in programme or project management. You will have experience working under your own initiative and prioritising a complex range of tasks. You will have excellent communication skills and the ability to lead a team of wellbeing facilitators. Key responsibilities include: To oversee the successful delivery of the project To manage a team of facilitators across the Dorset and Somerset area To ensure a quality service with defined and clear outcomes as per contract To resolve pathway difficulties and ensure smooth transition of patients from start to finish of the programme including early exit strategies.
