Diabetes Programme Facilitators (Contract) Westbank is delighted to have been awarded the contract to deliver Healthier You the pioneering Diabetes Prevention Programme for the NHS in Devon, Cornwall, Dorset and Somerset. The ground-breaking programme aims to reduce people’s risk of developing Type 2 diabetes by supporting people to make sustainable lifestyle changes that help them to better understand and to take control of their diet and exercise and to better manage their health. We are looking for Facilitators to work in the following areas: Devon (Excluding Torbay and Plymouth) Plymouth Torbay Dorset As a Facilitator, you will deliver group-based educational sessions across to support and motivate participants to take control of their health and reduce their risk of developing Type 2 Diabetes. The programme is run over 12 months and helps participants understand their risk of and the implications of Diabetes and make positive changes to lose weight, improve their diet and increase levels of physical activity. Key responsibilities include: Delivering the Living Well Taking Control curriculum to participants, ensuring they achieve and experience the outlined learning outcomes. Carrying out individual assessments with participants who have been referred to the programme by their GP practice Facilitating group sessions effectively and ensuring adherence to the programme.
