About the role:

We are looking to recruit a motivated and caring Hospital Liaison Worker who will become a proactive member of our Hospital Services team. You will play a crucial role in supporting family, friends and unpaid carers when either themselves, or the person they care for, has been discharged from hospital, by carrying out initial assessments with unpaid carers to establish needs arising from their caring role.

As a Hospital Liaison Worker:

  • You will support family, friends and unpaid carers (‘carers’) when either themselves, or the person they care for, has been discharged from hospital;
  • You will work to avoid admissions by supporting carers;
  • You will carry out initial assessments with carers to establish needs arising from their caring role, and using a strengths-based approach to work with them to support these needs;
  • You will deliver an intensive 1:1 wellbeing support service to carers for up to 6-weeks after discharge from hospital;
  • You will undertake Carers Assessments (Health & Wellbeing Checks) for carers who still have eligible needs under the Care Act at the end of the Hospital Services support;
  • You will carry out virtual and physical ward rounds within the acute hospitals in Devon, on a rota with others in the team.

Please Note: This role is mainly hybrid working, but includes weekly visits to either the RD&E, North Devon District Hospital, Torbay Hospital or Derriford Hospital (depending on location)

What we are looking for:

We will be shortlisting on the following criteria, so please make reference as to how you meet these in your covering letter, ensuring similar work examples are included

  • NVQ III, or equivalent, qualification in Health & Social Care, or another relevant field, or a willingness to work towards such a qualification;
  • Experience of providing one-to-one advice or support in a similar / relevant role;
  • Excellent communication and interpersonal skills, with the ability to empathise with the needs of unpaid carers;
  • Good knowledge / understanding of unpaid carers and their issues;
  • Ability to assess information, apply eligibility criteria, and plan appropriate actions and support to unpaid carers at risk of developing, or who have, eligible needs;
  • Ability to develop and maintain constructive relationships with unpaid carers and professionals from other agencies, and share information appropriately;
  • Ability to work as a member of a professional team to meet shared objectives;
  • Good keyboard and IT skills, including the use of MS Office packages.

Rate of pay: £14,931.82 pro rata - £23,019.89 FTE (increasing following successful probationary period)

Hours per week: Minimum 24 - 37 (working days to be agreed with successful candidate)

Location: Hybrid Working (with weekly hospital visits)

Contract: Fixed Term Contract until 31/03/2023
