Contract: This is a fixed-term contract to 30th April 2023

Our Lead Practitioners carry a caseload of the most complex Carers Assessments and support planning in their specialist area, in this role this will be Learning Disability. They ensure there is effective co-ordination/liaison when undertaking joint assessments and supporting carers to access specialist services.


Key responsibilities include

  • Acting as a focal point for the relationship between Devon Carers and relevant specialist assessment teams and services;
  • Providing relevant/specialist support for Carers Support Officers (CSOs) and other staff undertaking carers assessments and support planning that requires additional co-ordination/liaison.
  • Developing and maintaining relevant information, training and other resources to increase the skills and knowledge of Devon Carers staff and service users.
