Wellbeing Facilitator (Diabetes Prevention Programme) Rate of pay: £13,853 (£23,088 FTE) Hours per week: 22.2 Location: Gloucester and Cheltenham We are looking for a Facilitator to support the delivery of the Living Well Taking Control Programme, part of the Healthier You NHS Diabetes Prevention Programme, in Gloucester and Cheltenham. As a Facilitator, you will deliver group-based educational sessions to support and motivate participants to take control of their health and reduce their risk of developing Type 2 Diabetes. The programme is run over 12 months and helps participants understand their risk of and the implications of Diabetes and make positive changes to lose weight, improve their diet and increase levels of physical activity. Key responsibilities include: Delivering the Living Well Taking Control curriculum to participants, ensuring they achieve and experience the outlined learning outcomes. Carrying out individual assessments with participants who have been referred to the programme by their GP practice Facilitating group sessions effectively and ensuring adherence to the programme.
