Living Well Taking Control is a provider for the Healthier You NHS Diabetes Prevention Programme, which aims to reduce people’s risk of developing Type 2 diabetes. The aim is to support people to make sustainable lifestyle changes that help them better understand and to take control of their diet and exercise and to better manage their health. The programme is delivered in a group environment in community venues across a range of sites in the UK.

Shortlisting Criteria:

  • Experience of facilitating group sessions
  • Worked with a variety of communities, health care professionals and patients ideally in a health promotion environment
  • High ability to communicate with individuals at all levels and using various forms of communication
  • Ability to facilitate and engage with participants
  • Demonstrate drive, energy and enthusiasm for self-management principles, health promotion and long term conditions
  • Experience in a community involvement role
  • Knowledge and understanding of the principles of self- management and motivational interviewing.
  • High level of organisational ability
